SPORT4LIFE – VOLUNTEER PROGRAM Assistant Coach Volunteer Program (Ages 14–17) Waiver, Liability Release & Photo/Video Authorization ⸻ 1. Program Description The Assistant Coach Volunteer Program provides teens (ages 14–17) with opportunities to develop leadership, communication, responsibility, and coaching skills in a supervised sports environment as part of Sport4Life and BF Training Academy activities Program duration: 3 months (possibility of up to 30 hours) ⸻ 2. Eligibility To participate, the volunteer must: • Be enrolled in High School • Attend a public school • Be between 14 and 17 years old • Be able to arrive at training facilities independently • Have parent/guardian authorization ⸻ 3. Responsibilities of the Volunteer • Assist the coach before, during, and after practices • Follow BF Training Academy rules and staff instructions • Arrive early to sessions • Maintain respectful and professional behavior • Wear the Sport4Life volunteer uniform • Help organize equipment and maintain a clean environment • Assist with athlete check-in • Support training plans provided by the coaching team Volunteers must always act as positive role models. ⸻ 4. Benefits • Community Service Hours (eligible for Bright Futures program) • Resume experience: Assistant Coach Volunteer – Sport4Life • Development of leadership, confidence, discipline & communication skills • Possibility of recommendation letters based on performance • Coaches feedback • Class on basketball strategy, techniques and tactics with the coaches from BF Training ⸻ 5. Assumption of Risk & Liability Waiver By signing this document, you accept full responsibility for the health and safety of the minor participating. Participation in sports activities involves inherent risks, including but not limited to: • Accidents • Falls • Minor or serious injuries • Illness (including COVID-19 and other transmissible diseases) Although Sport4Life’s coaching supervision is reasonable, the Sport4Life training team cannot guarantee safety. I acknowledge that the volunteer’s participation is entirely voluntary. To the fullest extent permitted by law, I waive and release Sport4Life, BF Training Academy, their coaches, staff, partners, volunteers, facilities, and organizing entities (“Released Parties”) from any and all claims, damages, or liabilities connected to participation in the program, including those arising from negligence. I also agree not to sue the Released Parties and to indemnify them against any claims related to my child’s participation. ⸻ 6. Medical Authorization I certify that my child is in good physical health and able to participate. In the event of a medical emergency, 911 may be called, and I understand that I am fully responsible for all medical expenses. If my child develops fever, illness, or tests positive for COVID-19, I will not send them to training and will notify the program immediately. ⸻ 7. Photo/Video Authorization I grant permission to Sport4Life and its agents to capture and use photos, video footage, and statements of the volunteer for: • Publication • Promotion • Social media • Advertising • Educational content I waive the right to compensation or approval regarding such use. ⸻ 8.
